Utility Application & Instructions

To begin, click here to open the form. Once opened, please print the form, and complete as follows:

  1. Application date: Current Date. Mandatory.
  2. Deadline date: Not required when filling out online form.
  3. Name: Applicant’s full name. Mandatory.
  4. Owner / Renter Checkbox:  Please check only one. 
  5. Service Address: Sabina street address where service is requested. Mandatory.
  6. Billing Address: Street address where bill is to be sent. Mandatory.
  7. Sanitation Collection & Recycle:  Please check “Opt In” OR “Opt Out”.  Mandatory.  Check only one. Sanitation cost is $13.95 per month. Recycling is an additional $6.00 per month.
  8. Customer Transferring: Check and supply previous address ONLY if you are moving from one address in the Sabina Utility service area to another.
  9. Renter’s Phone Number: Mandatory if renting.
  10. Renter’s Signature: Mandatory if renting.
  11. Property Owner’s Signature: Residence Owner.  Mandatory.
  12. Property Owner’s Signature: Mandatory.
  13. Property Owner’s Address: Mandatory.

Once signed where appropriate, the form must be delivered in person by the applicant with photo ID.  If the applicant is a renting, the property owner must also sign. The property owner will be contacted to verify renter status. The Sabina Utility Office is located as 101 N. Howard St. (right side entrance of the Municipal Building).
